How do I schedule a physical appointment?

Physicals are scheduled at particular times in accordance with physician availability. Currently physicals are scheduled Tuesday and Wednesday at 8:15 am. There may be times during the calendar year when physical appointments may be restricted due to an increase of student illness in the SHS Primary Care Clinic.

SHS cannot perform Peace Corps physicals, Insurance physicals, flight physicals, or pre-employment physicals. Other specialized physicals, scuba diving and athletic, must be discussed with the nurse before scheduling the appointment. We can schedule ROTC, camp/work physicals, Turkish military physicals, and health maintenance physicals.

Steps to obtaining a Physical appointment:

1. Please access the Health History Checklist on this website. You will need to call 404 385-6398 to review the voice message on how to complete your paperwork to prepare for your visit.

2. Print the form, fill it out completely and fax it to the Nurses’ Station at 404 385-0717. You may also pick up a copy of the form from the front desk or the Nurses’ Station at SHS.

3. An RN will return your call to clarify your information and schedule an appointment for you.

4. If you are unable to keep the appointment, cancellation must be done 24 hours before the appointment.
