Information for Incoming Students
Immunization Requirements
All incoming students must comply with the University System of Georgia's and the Board of Regents' immunization requirement. This mandatory requirement must be met before registration. Proof of the following immunizations or immunity must be documented on the Georgia Tech Medical Entrance forms. Immunization and tuberculosis screening forms must be signed, dated, and addressed by a medical practitioner.
Medical Entrance Forms
Proof of immunization or immunity must be documented on the Georgia Tech Medical Entrance forms, which must be completed and signed by a medical practitioner. A copy of the forms can be downloaded from our Web site. (Click here)
All students must provide documentation for the following prior to attending classes:
- Immunization for measles, mumps and rubella(MMR)tetanus, varicella (chicken pox) and hepatitis B
- Tuberculosis Screening

Tuberculosis Screening
All students must be screened for tuberculosis before entering Georgia Tech. The tuberculosis screening form must be completed and signed by a medical practitioner.
MANTOUX Skin Test - If a skin test is required, a Mantoux skin test must be performed in the United States
within one year prior to the first day of classes. Skin tests
must be recorded in millimeters.
Tine Tests are not acceptable. Positive skin tests must be followed up by the student’s family physician or local health department prior to registration. Documentation of follow-up must be submitted to the Student Health Center. Documentation must include chest x-ray report and record of treatment advised or begun by your physician.
Failure to comply with this requirement will prevent registration.
It is recommended that all students make an additional copy of the all of the documentations and bring them along, in case the originals are lost or delayed in the mailing process.
