Clarification of Policy Regarding Requests for Excuses from Class

The purpose of this memorandum is to explain Stamps Health Services (SHS) position as it relates to class excuses and to have a simple form for Stamps Health Services’ staff to provide students in situations of requested proof of medical visit.
Stamps Health Services’ primary mission is to support students to achieve their greatest academic potential.
We also seek to educate, empower and challenge our students as adults to self manage their health and health related issues.
Class excuses are no exception.
Stamps Health Services believes that issues of attendance and class requirement are matters to be discussed and resolved between the student and instructor and any additional campus resources as required.
Any health center documentation to support a student’s absence must be obtained by the student and comply with privacy laws and Stamps Health Services policy for release of health information.
Stamps Health Services will provide health information directly to the students and it is at their discretion to share such information with any instructor upon request.
This includes this simple note displaying proof of visit to a medical provider at SHS.
It is Stamps Health Services intent to remain completely impartial in matters of student absence, respect privacy rights of the students, allow instructors to adjudicate absences, and only provide information upon the students request and written authorization.
