Are you a new student to Georgia Tech? Did you know that your class registration can be delayed if you don't have your immunizations properly documented?
Atlanta Campus
Follow these 3 steps to avoid registration delays!
- Download a copy of the required immunization forms (Atlanta Campus)
- Complete the immunization forms, being sure to include all required signatures from your doctor, and fax them to 404-385-0329. If you are attending FASET, it's best to fax your forms at least a week before your scheduled FASET session.
- Check the status of your immunization forms, Online Immunization Records.

Please allow 5 to 7 days for your submitted immunization forms to be processed. As such, you are encouraged to submit these forms at least one week before you are scheduled to attend FASET orientation or otherwise register for classes. |
