Don't Get Stung by a Registration Hold!

Avoiding a registration hold is as
easy as 1-2-3!

Step 1:
Complete health forms

Health Forms

Step 2:
Please mail or fax your completed forms

Stamps Health Services – Medical Records
Georgia Institute of Technology
740 Ferst Drive NW
Atlanta, Georgia 30332-0470
Fax: 404-385-0329

Step 3:
Verify your registration hold has been removed

**Electronic Verification via OSCAR Coming Soon**

Stamps Health Services is dedicated to processing your documents efficiently and effectively. We strive to have all registration holds removed prior to orientation.

However, please acknowledge that it is 100% your personal responsibility to confirm you do not have a registration hold.

US mail service, faxes and other means of delivery are not a guarantee of arrival and removal of your registration hold. Given the large volume of submission and variability of this process, the earlier you submit accurate and complete forms the better your chances are of not having a registration hold.

Please read this important information!

All health forms should be completed.
Some are required forms; one is recommended and one must be determined by a doctor or registered nurse.

The two page Certificate of Immunization Forms are documentation of your history of immunizations and must be verified and signed by you and your medical doctor or registered nurse. Page 1 is REQUIRED and Page 2 contains recommended immunizations and will not effect your registration hold, but please choose wisely.

Tuberculosis (TB) is a serious medical illness that can be contracted if exposed to a person with active Tuberculosis for prolonged periods of time. In order to protect the Georgia Tech community, we require all students to have a TB screening conducted by a medical doctor or registered nurse. If your doctor or registered nurse believes you've had a higher risk for TB exposure based upon the TB screening criteria (based upon Centers for Disease Control and Prevention guidelines) a second step, known as the TB SKIN TEST and FORM will be necessary.

** INTERNATIONAL STUDENTS**
Please note: TB SKIN TESTS WILL ONLY BE ACCEPTED IF ADMINISTERED IN THE UNITED STATES OF AMERICA.

Stamps Health Services at Georgia Tech is prepared to administer immunizations and TB skin tests to admitted students only during orientation. To avoid registration complications plan to arrive early in orientation week to the USA and Stamps Health Services.
